Grade 1, 2 or 3 fatty liver on ultrasound: what does it mean?
Short answer
The grade of fatty liver on an ultrasound describes how bright the liver looks, which roughly reflects how much fat it contains: grade 1 is mild, grade 2 moderate and grade 3 severe. The grade does not tell you whether the liver is scarred, which matters more. Blood tests and a FibroScan show that.
What the grades show
Grading is a visual judgement, so it can differ between machines and doctors, and it can change from one scan to the next.
- Grade 1: the liver is slightly brighter than normal (mild fat)
- Grade 2: moderately bright, with blood vessels in the liver less clearly seen
- Grade 3: very bright, with deeper parts of the liver hard to see
Why the grade is not the full story
The real risk in fatty liver is scarring (fibrosis), which can slowly lead to cirrhosis. A person with grade 1 fat can have scarring, and a person with grade 3 fat may have none. So the next step is to check liver enzymes, blood sugar, cholesterol and a FibroScan.
What to do
Fatty liver at any grade can improve, and often disappears, with these changes.
- Lose 7 to 10% of body weight slowly if you are overweight
- About 150 minutes of brisk walking or other exercise a week
- Cut down sugar, sweet drinks, maida and fried snacks
- Stop alcohol
- Control diabetes, blood pressure and cholesterol
When to see a gastroenterologist
See a specialist if liver enzymes stay high, a FibroScan shows scarring, you have diabetes with fatty liver, or there is jaundice, swelling of the belly or legs, or blood in vomit or stool.
Diet for fatty liver
Eat more
- Half the plate vegetables or salad, a quarter dal, curd, paneer, eggs or fish, and a quarter roti or rice
- Whole grains and millets: atta with chokar, bajra, jowar, ragi, brown rice
- Whole fruit in place of juice, and a small handful of nuts
Cut down
- Sugar, mithai, cold drinks, packaged juices and sweetened chai
- Maida: white bread, biscuits, bakery items, noodles
- Fried snacks, and large amounts of ghee, butter and oil
Good to know
- Fatty liver is common in slim people in India too; the waist size and blood sugar matter as much as weight.
- Try the Thali meter on the home page to check the fibre, protein and fat in your meals.
Common questions
Is grade 2 fatty liver dangerous?
Not by itself. The grade shows how much fat there is, not scarring. What matters is the FibroScan and blood tests, which show whether the liver is being damaged.
Can fatty liver go away completely?
Yes, very often. Losing 7 to 10% of body weight slowly, regular exercise and stopping alcohol can clear the fat and even improve early scarring.
Is ghee bad for fatty liver?
A small amount is fine. The total fat and calories in the day, and sugar and maida, matter more than any one food.
Are there medicines for fatty liver?
Some medicines help selected patients, especially those with diabetes, obesity or scarring, and your doctor will advise if they suit you. Lifestyle change remains the main treatment.
