What is a liver abscess, and how is it treated?
Short answer
A liver abscess is a collection of pus inside the liver. In India, many are caused by amoeba spread through contaminated food and water; others are caused by bacteria. It causes fever and pain in the upper right belly, and is treated with medicines and, when large, by draining the pus with a needle.
Symptoms
- Fever, often high, with chills
- Pain in the upper right belly, sometimes felt in the right shoulder
- Pain on breathing deeply
- Loss of appetite, weakness and weight loss
- Sometimes mild jaundice
Who gets it
Amoebic abscess is more common in men, in people who drink alcohol and where water may be unsafe. Bacterial abscess is more common in people with diabetes, gallstones or bile duct problems.
Tests and treatment
An ultrasound or CT scan shows the abscess, and blood tests help find the cause.
Most people recover fully. The abscess can take weeks to months to disappear on ultrasound, even when the person feels well.
- Amoebic abscess: anti-amoebic medicines, followed by a medicine to clear amoeba from the gut
- Bacterial abscess: antibiotics through a drip, then tablets, for several weeks
- Large abscesses, or those not improving, are drained with a needle or a thin tube under ultrasound guidance
- Blood sugar is controlled, and any bile duct problem is treated
When to seek urgent care, and prevention
Go to hospital straight away for severe pain, breathlessness, high fever with confusion, or sudden worsening, as a large abscess can burst.
Drink safe, boiled or filtered water, wash hands before eating, and avoid cut fruit and street food when hygiene is uncertain.
Food during recovery
Eat more
- Light, fresh home food: khichdi, dal-rice, curd, idli, soups
- Protein to rebuild strength: dal, paneer, eggs, fish or chicken
- Plenty of boiled or filtered water, coconut water and buttermilk
Cut down
- Alcohol, completely during treatment and ideally afterwards
- Street food, cut fruit and raw salads from outside
- Sugary foods if you have diabetes, which slows healing
Good to know
- Wash hands with soap before eating and after the toilet; amoeba spread through dirty hands and water.
- Some medicines for amoebic abscess cause a metallic taste or nausea; avoid alcohol completely while taking them.
Common questions
How long does a liver abscess take to heal?
Most people feel better within one to two weeks of treatment. The abscess can stay visible on ultrasound for months even when it is healing well, so the scan alone is not a reason to worry.
Does every liver abscess need draining?
No. Many amoebic abscesses settle with medicines alone. Large abscesses, those close to bursting, or those not improving after a few days are drained with a needle or tube.
Can it come back?
It can, if the source continues, such as unsafe water or an untreated bile duct problem. Completing the full course of medicines and the gut-clearing medicine lowers the risk.
